To enter an altar, you have to have a talisman or tiara for that specific altar. Talismans have to be carried in your inventory and tiaras can be worn on your head so you will be able to carry an extra essence with them. Talismans are found all over Runescape, mostly by fighting certain monsters. Tiaras can be crafted from silver. (Crafting Guide) Tiaras can be crafted by taking blank tiara and a talisman inside the altar and combining the two. You also gain Runecrafting experience for crafting tiaras.

Combination runes are members only and serve as a way to use less runes to cast more spells. To make combination runes, you will need to have a rune of one elemental type, a talisman which matches the type of elemental rune you have brought with you, and some pure essence. You should then head to the altar of the secondary element you wish bound into your combination rune. Use the talisman that matches the elemental runes you brought with you on the altar of the secondary element and you will attempt to bind the energies into a combination rune. There is a 50% chance that you will craft the combination rune, but if you wear a Binding Necklace, it will be a 100% chance.
